HKMS Spring Social Permission Slip & Payment Form

Dear All,
Our next Student Government is our Spring Social on Thursday, March 12th from 3pm - 4:30 pm!
We will have music and food in the cafeteria and games in the gym. In order for your child to attend this event, please fill out this permission slip, including paying the $10.65 fee. The fee includes admission, pizza and treats!
Please review the attached permission slip, sign, and submit with the $10.65 payment by Wednesday, March 11th at 11:59 pm. Permission slips and payment will not be accepted on the day of the social.
Once signed, you will receive electronic confirmation via email. Once paid, you will receive electronic confirmation via email. Reminder emails and texts are only sent to parents who have NOT signed the permission slip. Students without a signed permission slip and payment by the deadline will not be allowed to attend the social.
IMPORTANT PICK UP INFORMATION:
Please pick your child up PROMPTLY at 4:30pm
