Meningococcal Disease Sheet |

August 2011-
Student’s entering grade 7 will be required to have a
second dose of varicella vaccine, and a Tdap (tetanus) booster.
Those families who have already handed in grade 6 physicals, who are
missing vaccinations, have been notified by regular mail. |

August 2011-
Student’s entering grade 7, in addition to the above
vaccines, will also be required to have the Meningococcal Vacine
(MCv4) prior to entering school. Please follow the link
