Information:
The Health office
(Nurse’s Office) is open from 8:00 A.M. to 3 P.M., Monday through Friday,
the days that school is in session. Please call should you have any
questions about this website at 268-8651, ext. 6. The HKMS fax
number is 268-6105.
If the Health office
needs to close during the school day, students will be directed to
the main office.
Students are not
permitted to carry medication on their person or keep medication on
their person or locker while at school. Medications must be stored
in the Health office.

Vaccination Information
Very Important Notice to
parents of Grade 6 Students:
Per state
regulations, section 10-206, every grade 6 student must have a
physical exam, with any updated immunizations needed, during the
grade 6 school year. The physical must be performed between June 1
or your child’s 5th
grade year and the last day of school for the 6th grade. If
the assessment is not submitted by the last day of school in grade
6, the student may not be allowed to enter 7th grade. Follow this

Please be aware that doctor’s
appointments for routine, well physical exams, fill quickly
especially in the spring. You should phone your doctor’s office
early for an appointment allowing yourself enough time to get the
grade 6 physical completed in a timely manner.
Injured and/or chronically ill
students
Students who are injured
and/or that have a chronic illness and cannot participate in PE must
obtain a doctor’s note indicating the nature of the injury or
illness and the length of time that the student needs to be excused.
If your child is on crutches, the doctor’s note must include
permission for your child to be on crutches and use the elevator, if
necessary, while in school.
